About Forms

Forms are an easy and simple way to collect your respondents' background information. You can create a whole survey solely containing form-type questions or you can mix it up with the other question types.

Form fields are based on your survey background variables.

Create Forms

To add a Form question, all you have to do is:

  1. Go to your survey, click the add survey element icon (GreenPlusIcon.png), and choose question.
  2. Choose form and click next.
  3. Type in your heading and text and change the typography (bold, italic, and underline).

    Use the merge field dropdown menu to insert your respondents' background information or their answers to previous questions.

    The length of question text is restricted to max. 250 characters.

  4. Click add background variable.
  5. Since form fields are based on background variables, you have two options. You can add existing variables or you can create new variables and add them to the form. You can add three types of variables: text, selection list, or date.
      1. Select existing variables.
      2. Select the variable(s) you want to add.
      3. Once you're done adding the variables as form fields, click add.

      Text variables are perfect to collect information such as email, zip code, name, etc. To add a text variable, all you have to do is:
      1. Select text.
      2. Give it a name and define the validation. Text variables can have a text, numeric, email, or custom format.
      3. When you're done, click add.
      4. The form field has now been added to your form question and your survey.

      This option will give the respondent options to choose from, e.g. a list of age brackets or countries. To add a selection list variable, all you have to do is:
      1. Select selection list.
      2. Give it a name and define the validation.
      3. Click add response options... to add options to the variable.
      4. When you're done, click add.
      5. The form field has now been added to your form question and your survey.

      The form field allows the respondent to select a specific date from a calendar widget.
      1. Select date.
      2. Give it a name and define the validation.
      3. When you're done, click add.
      4. The form field has now been added to your form question and your survey.

      Your account language determines the date format of your download. The format for accounts in the English language is mm.dd.yyy. All other languages are dd.mm.yyyy.

  6. You can move and delete the fields by using the actions menu.

    Deleting the fields from the form, will not delete them from the survey. Learn how to delete background variables.

  7. When you've added all the form fields and everything looks good, click add.
  8. Your form will be added immediately. Though it can contain one or several background variables, it will appear as one element in the survey content overview.

Background variables are unique, which means that if you use the same one in several forms, whichever variable is inserted last will overwrite the previous data entries. Instead, opt for creating new variables per form.

If you are using the system variables first name, last name, and/or email in your Form question, make sure that the survey is not anonymous. These system variables are anonymized in anonymous surveys and will therefore not be accessible in your response data download.

Forms in Response Data Downloads

You can download your survey responses from Enalyzer for several purposes. You can use the file for analysis in other software, share it with your team, or import it to another Enalyzer survey.

Since forms can consist of one or multiple background variables of different types, there isn't one single format for form questions in response data downloads. It all depends on the background variables being used. Learn more about the different background variable types in response data downloads:

In response data downloads, background variables will be shown in order after the system variables and before any survey questions.
